Attention 2007-2008 Athletes |
|
All
student-athletes, new and returning,
need to follow the forms link
above and print out all the
necessary forms prior to
participation in Mountaineer
athletics.
All
athletes must submit proof of
primary health insurance, preferably
a copy of the front and back of your
card. If obtaining health
insurance through the University
Health and Wellness Center, a copy
of the receipt is adequate until a
card is obtained.
*Please note that submitting proof
of primary health insurance to the
athletic department is separate from
submitting it to the University
Health and Wellness Center. |
