All student-athletes are required to print, complete, and turn the below forms into the athletic training room.  Athletes will not be allowed to participate in Schreiner University athletics until all forms are submitted.

Pre-participation physicals are required for all student-athletes who have not participated in Mountaineer athletics in the past.  Athletes are encouraged to see their primary physicians for a physical prior to arrival on campus.  However, the athletic department hosts two dates for physicals with our team physician, Dr. Clint Morris. The first date is prior to classes starting for fall athletes.  The second, for all other student-athletes, is scheduled in the beginning of the fall semester.  After this time, student-athletes are responsible to set up their own physical through the University Health Services and Dr. Noonan or another physician.
